Job summary

Deercreekseniorliving is seeking a skilled Registered Nurse to perform nursing visits for clients of all ages in their homes. Responsibilities include admissions, patient care coordination, and supervision of hospice visits.

Must have a Texas RN license, one year of clinical experience, and reliable transportation. The job requires adaptability to various conditions and a flexible schedule.

Qualifications

• Current Texas State license as a Registered Nurse.
• One year experience as a nurse in a clinical care setting.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Perform skilled nursing visits including admissions and administer nursing care.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Admit, Recert, SCIC, Post-hospital and Discharge visits.
• Supervisory hospice visits to patients.

SUMMARY

Primary functions are to perform skilled nursing visits including admissions and administer nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assume the responsibility for coordination of care.

DUTIES AND RESPONSIBILITIES

• Skilled visits as assigned.
• Admit, Recert, SCIC, Post-hospital and Discharge visits as assigned.
• Supervisory hospice visits to patients.
• Preparing home health aide care plans.
• Check charts for completeness and accuracy.
• Attend staff meetings.
• On-call rotation.
• Other assigned duties.

JOB QUALIFICATIONS

• Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Licensure: Current Texas State license as a Registered Nurse, current Texas Driver’s License.
• Experience: One year experience as nurse in a clinical care setting.
• Skills: Nursing skills as defined as generally accepted standards of practice; Knowledge of State and Federal regulations; Good interpersonal skills.
• Transportation: Reliable transportation; valid and current auto liability insurance.

ENVIRONMENTAL AND WORK CONDITIONS

• Works in patient’s home in various conditions and locations; possible exposure to blood and bodily fluids and infectious diseases.
• Proof of current CPR, and Hepatitis consent/declination.
• Ability to work flexible schedule.
• Ability to travel locally.
• Some exposure to unpleasant weather.
• PRN emergency call.
