Job summary

Assured Hospice, part of the LHC Group and UnitedHealth family, is seeking a Per Diem RN Case Manager to deliver comprehensive hospice care. You will evaluate and manage the Plan of Care for each patient, ensuring timely assessments and high-quality care within the hospice framework.

Responsibilities include initial eligibility evaluations, regular re-assessments, and collaborative care planning with the patient, family, and care team, while adhering to hospice policies and objectives.

Qualifications

• RN licensure in the state of practice.
• 1+ years of clinical experience.
• Valid driver's license or access to dependable transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identifies patient's/family's physical, psychosocial, and environmental needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician and IDG members for daily care.

As the Per Diem Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluate personal and professional performance and makes necessary changes to increase productivity and quality of care delivered. Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care.

Primary Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral
• Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days
• Initiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition
• Documents problems, appropriate goals, interventions, and patient/family response to hospice care
• Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily

Required Qualifications

• Current unrestricted RN licensure in the state of practice
• 1+ years of clinical experience
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation

Preferred Qualification

• Current CPR Certification or ability to complete within 90 days of hire

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $86,016 to $129,024 annually based on full-time employment. We comply with all minimum wage laws as applicable.
