Job summary

Redcrest Hospice in Muncie, IN is seeking a Registered Nurse (RN) for PRN shifts across weekdays, overnights, weekends and holidays. You will provide compassionate end-of-life care, working one-on-one with patients and families as part of an interdisciplinary team.

You will follow physician directions, document according to agency and Medicare guidelines, and support the Plan of Care. The role demands autonomy, attention to safety, and the ability to travel within Muncie and surrounding areas.

Qualifications

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting is preferred.
• Hospice experience is desired.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.

Responsibilities

• Plan of Care: evaluate the patient’s treatment and medical condition per the plan.
• Individualize the plan of care and adjust as needs change.
• Minimize patient risk: update orders and administer meds as prescribed.
• Continuity of care: coordinate multi-disciplinary care and patient transitions.
• Family educator/advocate: counsel patient and family in plan of care.
• Listen to patient and family members to address concerns.
• Complete clinical documentation following agency protocol and Medicare guidelines.
• Oversee total care of patient by nursing staff and aides; participate in trainings.

You will utilize your clinical skills in providing end of life care as directed by an attending physician. HOW YOU'LL WORK:

You’ll oversee the overall care of the patient from beginning to end of life. You will observe and monitor the patient to determine if the level of care being provided meets the patients’ needs. You will work autonomously and be responsible for all practices and duties within the scope of practice outlined by the state.

MAJOR AREAS OF RESPONSIBILITY:

• Plan of Care: Evaluate the treatment and medical condition of the patient according to the plan of care.


• Individualize the plan of care and make adjustments as needs change.


• Minimize Patient Risk: Update all doctor orders in patient chart and administer medication as prescribed by the physician.


• Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.


• Family Educator/Advocate: Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care.


• Listen to patient and family members.


• Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.


• Operations: Oversee and supervise total care of patient provided by nurse aides and LPNs.


• Attend in-service training and mandatory agency meetings.
