UnitedHealth Group is seeking a Per diem Registered Nurse Case Manager to deliver hospice care and manage the Plan of Care for patients in the East Valley. The role requires a current RN license and clinical experience, with travel to patient sites as needed.

Responsibilities include initial evaluations within 48 hours of referral, ongoing reassessment, and collaboration with families and physicians to ensure quality, compliant care. This position offers flexible hours and competitive hourly pay.

Qualifications

  • Current unrestricted RN licensure in the state of practice.
  • 1+ years of clinical experience.
  • Current CPR certification or ability to complete within 90 days.

  • Documents problems, appropriate goals, interventions, and patient/family response to hospice care
  • Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily

As the Per Diem Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluate personal and professional performance and makes necessary changes to increase productivity and quality of care delivered. Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care.

Required Qualifications:
  • Current unrestricted RN licensure in the state of practice
  • 1+ years of clinical experience
  • Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
Preferred Qualifications:
  • Current CPR Certification or ability to complete within 90 days of hire

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $39.40 to $59.10 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
