Job summary

Housecall Providers is seeking a Registered Nurse for hospice care in Oregon. This 0.8 FTE role includes night shifts on a seven-on/ seven-off schedule, with responsibility for comprehensive patient assessments, care planning, and coordination of interdisciplinary care teams.

The RN will document in the EMR, maintain compliance with Medicare and HIPAA, and collaborate with hospitals, facilities, and community resources to meet patient needs across the care continuum.

Qualifications

• Performs initial and ongoing nursing evaluations to develop and implement the plan of care.
• Coordinates care with IDT and communicates changes in patient condition to improve quality of life.
• Maintains documentation in the EMR in compliance with policies and regulations.

Responsibilities

• Perform initial and ongoing nursing evaluations of patients and families.
• Develop and implement the patient/family care plan with interdisciplinary input.
• Coordinate admissions, triage, and night/weekend coverage as needed.
• Document accurately in EMR and ensure HIPAA compliance.
