Job summary

Enhabit Home Health & Hospice in Illinois is seeking an experienced registered nurse to join our team. You will coordinate care, assess patient and family needs, and implement the interdisciplinary plan of care.

You will perform services per physician orders, ensure regulatory compliance, and maintain dependable transportation for home visits. A valid RN license in Illinois, CPR certification, and at least one year of home health or hospice experience are preferred.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Medicare home health or hospice experience is preferred.

Responsibilities

• Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan.
• Perform services in accordance with the physician's orders and the established plan of care (POC).
• Ensure delivery of care is compliant with the federal and state regulations.
