Job summary

BAYADA Hospice seeks a compassionate, full-time Registered Nurse (RN) to join our hospice team in Wilmington, Delaware. This role involves traveling to clients’ homes to provide palliative care and administer medications while coordinating with a multidisciplinary team.

Responsibilities include making home visits, documenting care, and following physicians' orders. Prior home care or hospice experience is preferred; strong EMR skills and reliable travel are essential.

Qualifications

• Current DE RN license.
• Minimum of one year of clinical (medical/surgical) experience.
• Home care and/or hospice experience preferred, but not required.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medications, wound care, treatments, and comfort care.
• Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
• Follow up with, execute, and document doctors' orders.
• Perform client assessments as necessary.
• Occasional on-call after hours and/or weekends.
