Job summary

First Call Hospice is staffing Registered Nurses to deliver compassionate home-based hospice care across the Greater Sacramento area. The RN plans, coordinates, and directs care in patients' homes, applying nursing processes and palliative principles to meet individual family needs.

In this role you will complete comprehensive assessments, develop care plans, administer prescribed treatments, educate families, and coordinate care as a case manager for an assigned caseload.

Qualifications

• Active California RN license is required.
• Hospice experience is preferred.
• Ability for prolonged walking/standing.
• Ability to lift, position and transfer patients.
• Maintains a current CPR certification.

Responsibilities

• Complete initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illness(es).
• Provide professional nursing care by utilizing all elements of nursing process.
• Assess and evaluate patient status and develop a care plan with palliative actions.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family in meeting nursing needs.
• Coordinate patient care as Case Manager for assigned caseload.
