Qualifications

• Must hold an Alabama RN license.
• Minimum 2 years full-time RN experience in oncology, acute care, long-term care, hospice or home health.
• CPR certification required.
• EMR knowledge required.
• Hospice/home health experience preferred.

Responsibilities

• Perform initial and ongoing patient assessments.
• Develop and implement the Hospice Plan of Care.
• Educate patient and family on Plan of Care, meds, dressing changes, oxygen and hospice philosophy.
• Act as liaison between physician, patient/family and hospice team.
• Maintain accurate and updated medical records.
• Participate in agency on-call schedule.
• Adhere to policies, procedures, laws and compliance guidelines.
