Job summary

Enhabit Home Health & Hospice is seeking a qualified clinician to coordinate care for patients in the home setting. You will assess needs, implement the interdisciplinary plan, and ensure services align with physician orders.

The role emphasizes compliance with federal and state regulations, driving quality of care, and collaborating with families. A state license and at least one year of professional experience are required.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
• Ensure delivery of care is compliant with the federal and state regulations.
