Job summary

Brighton/Suncrest Hospice is seeking a Registered Nurse to provide in-home and facility-based hospice care as part of an interdisciplinary team. You will deliver patient-centered care, perform psychosocial assessments, and coordinate with nurses, social workers, chaplains, and volunteers.

The role requires travel across the service area, valid driver’s license, and ability to work Tuesday through Saturday 8:00 a.m. to 4:30 p.m. EMR experience and strong time-management skills are essential.

Qualifications

  • RN licensure in good standing in state(s) of practice
  • Ability to collaborate with nurses, social workers, chaplains, volunteers and CNA leadership
  • Ability to travel across the service area with reliable transportation
  • Availability Tue-Sat 8am–4:30pm
  • Experience with electronic medical records system

Responsibilities

  • Provide care to a group of patients visiting their residences (homes, SNFs, assisted living)
  • Collaborate with interdisciplinary team in hospice care
  • Serve in Case Management, Admissions, or Resource role
