Job summary

AccentCare, Inc. is seeking a dedicated Hospice Registered Nurse (RN) to join our interdisciplinary team in Minnesota.

The role emphasizes delivering compassionate, patient-centered care with autonomy to practice at the top of the license while supported by a mission-driven organization. Responsibilities include developing and carrying out individualized care plans, collaborating with physicians and families, and participating in on-call rotations.

Qualifications

• RN license in the state of practice
• At least one year of nursing experience
• Reliable transportation, valid driver’s license, and appropriate auto insurance

Responsibilities

• Provide individualized nursing care to maximize comfort and quality of life
• Partner with patients, families, physicians, and the interdisciplinary team to implement care plans
• Perform comprehensive assessments—physical, emotional, social, and spiritual to guide care decisions
• Educate and support patients and caregivers, meeting them where they are
• Coordinate care across disciplines to ensure continuity and dignity
• Document care clearly and accurately reflecting patient needs and progress; participate in on-call or admission rotations as needed
• Engage in ongoing learning and professional development
