Job summary

VIA Health Partners in Clinton, SC seeks a Registered Nurse to coordinate and deliver patient and caregiver care in IPU/hospice settings. You will perform nursing assessments, implement and update care plans, educate families, and ensure continuity of care.Qualified candidates hold a Nursing Diploma/ADN (BSN preferred), active RN license, CPR, and 1–2 years of related experience; CHPN preferred. Strong computer skills and dedication to compassionate care are required.

Qualifications

• Diploma or ADN in nursing required; BSN preferred.
• Current, active RN license (compact multistate) required.
• CPR certification required; CHPN preferred.
• Minimum 1 year in IPU; 2 years for other nursing roles; hospice/palliative care experience valued.
• Proficiency with electronic health records/COB documentation.

Responsibilities

• Delivery of patient care and coordination with caregivers.
• Conduct nursing assessments and manage plans of care.
• Educate patients and families; ensure continuity of care.
• Maintain infection control and universal precautions.
• Communicate with IDG and health professionals for care coordination.
• Maintain licensure, CPR, and CEU requirements.
