Job summary

Caretenders Hospice is seeking an RN Case Manager to deliver in-home hospice care and manage the Plan of Care for each patient. You will collaborate with physicians and the IDG to implement strategies promoting physical, psychosocial, and spiritual well‑being.

Current Ohio RN licensure and a valid driver's license are required. The role involves travel within a 50‑mile radius, typical business hours, and potential on-call rotations. PRN options may apply for home care experience.

Qualifications

  • Current and unrestricted RN licensure in the state of practice (Ohio).
  • Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transport.
  • Experience in home health or hospice care is preferred.

Responsibilities

  • Conduct initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
  • Identify physical, psychosocial, and environmental needs and reassess at least every 15 days.
  • Initiate and coordinate the patient’s plan of care with the IDG.

As the Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered. You'll also develop the patient's Plan of care in collaboration with other professionals and implement treatment strategies based on scientific nursing theory that promotes physical, psychosocial, emotional, and spiritual well-being. Familiar with the concepts and needs of patients/families who are facing death and dying.

  • Full-time position providing in-home visits for hospice patients.
  • Current Ohio RN certification is required.
  • Annual salary range: $79,600 - $94,000
  • Standard business hours are Monday-Friday, 8:00 AM-4:30 PM.
  • Oncall rotation for evening/weekends as needed.
  • Travel required within a 50-mile service radius to provide patient care
  • PRN also available - Home Care experience required!
Primary Responsibilities:
  • Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral
  • Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days
  • Initiates and coordinates the plan of care
  • Documents problems, appropriate goals, interventions, and patient/family response to hospice care
  • Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily
  • Instructs and supervises the patient/family in self-care techniques when appropriate
  • Maintains accurate and relevant clinical notes regarding the patient's condition
  • Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualifications:
  • Current and unrestricted RN licensure in the state of practice
  • Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
Preferred Qualifications:
  • 1+ years of clinical experience
  • Current CPR certification or ability to complete within 90 days of hire

    Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. The salary for this role will range from $79,600 to $94,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
