Aspirus At Home in Wisconsin Rapids/Stevens Point, WI is seeking a Hospice RN Clinical Liaison to join our field team. The role focuses on increasing face-to-face discussions with patients and families about Home Health and Hospice services, collaborating with medical directors and care teams to determine eligibility and support timely discharges.

The RN must have an Associate degree in nursing (Bachelor’s preferred), be licensed in WI or MI, and have hospice experience.

Qualifications

• Associate degree in nursing is required; Bachelor’s in nursing preferred.
• One year of hospice experience required; two years preferred.
• Valid Wisconsin or Michigan driver’s license required.
• Automobile liability insurance with minimum $100,000/$300,000 coverage.
• Ability to collaborate and work efficiently with others.
• Strong patient advocacy skills.
• Ability to review clinical documentation for accuracy and completeness.
• Strong communication with individuals and groups.

Responsibilities

Aspirus At Home in both Wisconsin Rapids/Stevens Point, WI is seeking a HOSPICE RN CLINICAL LIAISON to join our field team!

The Clinical Liaison collaborates with the Hospice Medical Director, Palliative Care team, Home Health or Hospice leadership team, and inpatient providers to determine home health or hospice eligibility for our patients within the Aspirus system hospitals. Works closely with the care coordination team to ensure an appropriate and timely discharge for Hospice and Home Health patients. The primary focus/responsibility of this position is to increase face to face conversations with patients and families regarding Home Health and Hospice Services and the benefit to their quality of life. This position requires an understanding of the importance of home health and hospice goals and the impact they have on system initiatives.

HOURS: Full Time 1.0 FTE, 80 Hours Biweekly

Experience/Qualifications

• Associate degree in nursing is required. Bachelor's degree in nursing preferred.
• One year of hospice experience required. Two years of hospice experience preferred. Home health experience is beneficial.
• Possesses a valid Wisconsin or Michigan Driver's License required.
• Possesses automobile liability insurance with minimum $100,000/$300,000 coverage required.
• Ability to meet, collaborate and work efficiently with others.
• Strong patient advocacy skills.
