Job summary

Moments Hospice is seeking an RN Clinical Manager/Team Director to lead an interdisciplinary care team, ensuring holistic, compliant patient care. You will collaborate with nurses, social workers, chaplains, and volunteers to coordinate services for patients and families during end-of-life journeys.

You will supervise staff, improve care policies, and participate in after-hours coverage. A valid RN license, 3+ years of healthcare experience, and a driver’s license are required.

Qualifications

• RN license required with at least 3 years healthcare experience, hospice preferred.
• Leadership or management experience is a major plus.
• Valid driver’s license is required.
• Bachelor’s degree strongly preferred.

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings, documenting patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling, performance reviews, and foster a positive work environment.
• Act as the central hub for communication with families, physicians, and other providers.
• Analyze quality metrics and manage a caseload of patients as needed.
• Participate in after-hours and on-call duties to ensure seamless care.
• Maximize resource utilization and promote a collaborative work culture.
