PruittHealth Hospice is seeking a Hospice Registered Nurse (RN) Case Manager for PRN assignments in North Carolina. You will provide compassionate hospice nursing care in patients’ homes or care facilities, conducting evaluations, assessments, and treatments per individualized care plans.

Responsibilities include maintaining accurate documentation, adhering to policies, and ensuring continuity of patient care.

Qualifications

• RN license required and current, active with no restrictions.
• Home health or hospice nursing experience preferred.
• Excellent documentation and assessment skills.

Responsabilités

• Complete evaluations, assessments, and treatments per care plan.
• Maintain timely and accurate documentation of patient records.
• Follow policies and procedures related to hospice scope of practice.
