Job summary

Ascend Hospice is seeking a dedicated Hospice Registered Nurse Case Manager (RN) in Overland Park, KS. The role covers on-call, case management, and visits to ensure continuous, well-coordinated patient care. A 3k signing bonus is offered along with comprehensive benefits.

You will assess patients, plan and implement nursing care, and collaborate closely with the healthcare team and families to support end-of-life needs. Travel is required and driving is part of the job.

Qualifications

• Associate's Degree in Nursing is required.
• RN license must be active in state.
• Minimum 2 years of nursing experience; hospice experience preferred.
• Valid driver's license and reliable transportation
• Willing to travel
• Strong verbal and written communication skills

Responsibilities

• Complete an initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluate patients' nursing needs and adjust care plans.
• Use data to determine care plan requirements and priorities.
• Collaborate with the healthcare team, patient, and family to coordinate care.
• Counsel patient and family on end-of-life needs.
• Initiate appropriate palliative nursing procedures and report changes to physician.
• Supervise Hospice Aides and support staff.
