MaineHealth in Rockport, ME is seeking a Registered Nurse (RN) - Hospice Case Manager to deliver in-home nursing care for terminally ill patients, supporting families with compassion and dignity. This full-time, exempt role offers day shifts and the opportunity to make a meaningful difference in the community.

Qualifikationen

• Education: Graduate of an accredited School of Nursing required; BSN preferred.
• License/Certifications: Current applicable state(s) license as a Registered Professional Nurse required. Current BLS certification required or must obtain within 30 days of start date. Valid Driver’s License to operate a motor vehicle, with vehicle available for work covered by liability insurance as specified by agency policy required
• Experience: One year of recent RN experience required. Automatic advancement from RN I level if all onboarding requirements are complete, including the New Grad Residency (where applicable).

• Perform comprehensive patient assessments in patients' homes and coordinate care with the interdisciplinary team.
• Manage symptoms, administer treatments, and provide patient and family education.
• Collaborate with physicians, social workers, home health aides, chaplains and other team members to deliver compassionate, individualized care.
• Support independence and meaningful connections for patients and families throughout the hospice journey.
