Job summary

Dover Health is seeking a Hospice RN Case Manager to connect with patients in their homes, promoting physical, emotional, and personal well-being. You will collaborate with an interprofessional team to create individualized care plans and educate families on home care and medications.

The role includes supervising LPNs and Hospice CNAs and requires travel within the service area. Qualifications include a nursing degree, an active RN license, CPR certification, and willingness to obtain hospice

Qualifications

• Diploma, Associate or Bachelor’s degree in nursing.
• Current RN license, state-specific.
• Hospice or home care experience strongly preferred.
• Willing to obtain hospice and palliative care certification within 1 year.
• Current CPR certification.
• Current and valid driver’s license, reliable transportation, and current liability auto insurance.
• Ability to work well with others and take direction from management, and take initiative.
• Self-motivated with accountability for results.
• Proficiency in Internet browsers and MS Office including Outlook, Excel, Word and Publisher; familiarity with EHR systems.

Responsibilities

• Provide nursing care as prescribed by the physician within nursing standards and laws.
• Perform initial/comprehensive nursing evaluation and develop a care plan with the team and family.
• Regularly assess patient needs and report changes; intervene when necessary.
• Educate patients and families on illness management and home care needs.
• Participate in safety, quality, and regulatory activities.
• Pursue professional development and lifelong learning.
• Supervise LPNs and Hospice CNAs.
• Travel within service area to patient homes and care settings.
• Maintain flexibility to take on additional duties as assigned.
