Job summary

Arc Hospice & Palliative Care is seeking a full-time Hospice RN Case Manager in Ocala, FL. In this role, you will ensure high-quality, compassionate care for hospice patients while coordinating with an interdisciplinary team. Responsibilities include conducting assessments, developing care plans, and providing skilled nursing care.

The ideal candidate will be a graduate of an accredited nursing program and hold a valid RN license. The salary range for this position is $80,000 - $90,000 USD annually.

Qualifications

• Graduate of an accredited nursing program; Associate degree or higher preferred.
• Current RN license in the state of practice or valid multistate compact license.
• Minimum one (1) year of hospice, palliative care, or home health experience preferred.

Responsibilities

• Complete initial and ongoing assessments of patients’ needs.
• Coordinate with physicians for certification of terminal illness.
• Educate patients and families on disease progression and symptom management.
