Job summary

PruittHealth Hospice is seeking a Hospice Flex Registered Nurse RN Case Manager to join our team. This role supports patient care in home or care facility settings, delivering evaluations, assessments, and treatments per individualized care plans, while ensuring thorough documentation and continuity of care.

The RN must hold an active, unrestricted license in the state of practice and will contribute to a compassionate, patient-centered hospice program within the PruittHealth family.

Qualifications

• Must hold current, active RN license in the state of practice.

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan.
• Complete all documentation and records in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.
