PruittHealth Hospice is seeking a Hospice Registered Nurse (RN) Case Manager on an hourly basis to provide compassionate, patient-centered care in the patient’s home or a care facility in North Wilkesboro, NC. You will complete evaluations, coordinate treatments, document promptly, and maintain continuity of care under established policies to support patients and families.

• Current, active RN licensure in the state of practice.

• Maintain continuity of patient care in a hospice setting
