Job summary

PruittHealth Hospice is seeking a Registered Nurse Case Manager (RN) to provide compassionate hospice care in patients' homes or facilities across Onslow or Carteret Counties. You will perform evaluations, coordinate care, and document in accordance with policies while maintaining continuity of patient care.

We offer a $15,000 sign-on bonus, top‑of‑market pay, flexible schedules, and comprehensive health plans.

Qualifications

• Current, active RN license in the state of practice.
• Hospice/home-health experience preferred.
• Ability to coordinate complex patient care plans and communicate with families.

Responsibilities

• Perform evaluations and develop care plans per policy.
• Complete timely, accurate documentation and records.
• Follow policies and procedures related to scope of practice.
• Maintain continuity of patient care in a hospice setting.
