Moments Hospice is seeking an RN Case Manager to serve as the clinical lead for patients and families, coordinating care and managing symptoms in a field-based capacity. The role emphasizes compassion, education, and collaboration with a multi-disciplinary team.

Responsibilities include comprehensive assessments, care planning, medication administration, and family education. A valid RN license and EMR proficiency are required, along with reliable transportation.

Qualificações

• Active RN license in the state you work in
• Proficiency with electronic medical records (EMR)
• Strong organizational, communication, and critical-thinking skills
• Ability to work independently while maintaining high standards of care
• A compassionate, patient- and family-centered approach
• Valid driver's license and reliable vehicle with current insurance

Responsabilidades

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care
