Job summary

Moments Hospice in Missouri is seeking an RN Case Manager to lead the clinical care team for patients and families in the field. You will manage symptoms, coordinate care, and provide guidance during end-of-life transitions.

The role offers a competitive salary of $75,000–$90,000 per year, a fleet car program for business and personal use, and a full benefits package including medical, dental, vision, life, disability, generous PTO, and a 401(k) with company match.

Qualifications

• Active Registered Nurse (RN) license in the state you work in.
• Proficiency with electronic medical records (EMR)
• Strong organizational, communication, and critical-thinking skills
• Ability to work independently while maintaining high standards of care
• A compassionate, patient- and family-centered approach
• Valid driver's license and reliable vehicle with current insurance

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care
