Job summary

Transitions Hospice - Michigan is seeking an RN Case Manager in the Great Lakes Bay Region, Frankenmuth area to coordinate care and lead the interdisciplinary team.

You will conduct assessments, develop personalized care plans, administer medications per orders, document accurately, and educate families while participating in on-call rotations.

We offer competitive pay ($40-$46 per hour), comprehensive benefits, 401(k) with match, company-provided equipment, and PTO; active RN license required.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related experience preferred.
• Strong clinical judgment and independent field work ability.
• Excellent communication with care teams and families.

Responsibilities

• Conduct comprehensive ongoing assessments to understand patient needs.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Maintain accurate, timely clinical documentation.
• Serve as communication bridge between families, physicians, and community partners.
• Educate patients and caregivers to ensure confidence and support.
• Participate in on-call rotation per team policy.
