Wentworth Hospice, Amedisys is seeking an RN Case Manager to coordinate compassionate hospice care by assessing patients and partnering with the interdisciplinary care team to develop plans that meet each patient’s needs. The role includes on-call duties, referrals, and maintaining patient progress records.

Responsibilities include teaching patients and families, communicating with the care team, and documenting outcomes to support high-quality care.

Qualifications

• RN license required in the state(s) you work
• Current CPR certification required
• Valid driver's license and reliable transportation

Responsabilités

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so
• Maintains individualized patient care progress records with accuracy and timeliness
• Records pain/symptom management changes/outcomes as appropriate
• Teaches patients, families, and caregivers appropriate care techniques
• Regularly communicates with clinical manager and care team
• Performs on-call responsibilities and on-call services to patients/families as assigned
• Other duties as assigned
