TRU Community Care is seeking a Hospice RN Case Manager to travel to patient homes and facilities in the Longmont and Louisville areas. The role focuses on professional, compassionate care with a hands-on, patient-facing approach. Schedule is Monday-Friday, 8am to 5pm.
The RN will assess patients, coordinate with physicians, and support families through medical, emotional, and spiritual aspects of care. A valid Colorado RN license and reliable transportation are required.

Title: Hospice RN Case Manager
Location: Facilities in the Longmont and Louisville areas, primarily. In patient homes and facilities, in the community.
Schedule: Monday-Friday, 8am to 5pm
Pay: $88,067 to $105,680 per year
Sign-On Bonus: $6,000
The RN Case Manager offers professional care with a personal touch by focusing on each unique patient and their families, while providing medical, emotional, and spiritual support. The RN will travel to patient homes in their assigned area to provide appropriate care. This is a very hands‑on, patient facing role.
