Job summary

Amedisys is hiring a Hospice RN Case Manager in the Greenville, NC area. You will assess patients, coordinate with the care team, and implement individualized plans to ensure comfort and quality of life at home.

Responsibilities include care planning, on-call support, and education for patients and families. RN licensure, CPR, and a valid driver's license are required, with mileage reimbursement and comprehensive benefits provided.

Qualifications

• Minimum 1 year of nursing experience.
• Current RN license, state-specific.
• CPR certification.
• Valid driver's license with reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Maintains individualized patient care progress records with accuracy and timeliness.
• Records pain and symptom management changes/outcomes as appropriate.
• Educates patients, families, and caregivers on care techniques.
• Communicates regularly with clinical manager and care team.
• Performs on-call responsibilities for patients and families as assigned.
• Other duties as assigned.

• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

Qualifications

• One year of nursing experience.
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
