Compassus in California is seeking a Registered Nurse to plan, direct, and deliver hospice care in home settings. The role emphasizes collaboration with the IDG, comprehensive assessments, and care planning to meet patient and family needs.
Requires valid California RN license with preference for 1 year hospice/home health experience. Responsibilities include patient education, care coordination, and detailed clinical documentation in a supportive, on-site home health environment.

The Registered Nurse plans, organizes and directs Hospice care and is experienced in nursing, with an emphasis on community health education / experience. The Hospice nurse builds from the resources of the Interdisciplinary Group (IDG) to plan and direct services to meet the needs of individuals and families within their home environments. May provide patient care to premature infants, infants, pediatrics, adolescents, young, middle and older adults.
California (CA) Required: Education sufficient to meet certification, licensure, or registration requirements. Preferred: 1 year of recent experience in hospice, palliative care, home health, acute care, and/or skilled nursing as a registered nurse.
Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage. Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications. Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy. Other Skills and Abilities: Excellent observation, verbal and written communication, problem solving and basic math skills. Visual acuity and hearing to perform required nursing skills. Preferred working knowledge with computer applications, i.e. Microsoft Word/e-mail.
National Provider BLS – American Heart Association (Vendor Managed) Active state RN license (Vendor Managed).
Other Required upon hire: Active Registered Nurse License (Vendor Managed).
The demands of this role necessate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
