As the Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered. You will also develop the patient's Plan of Care in collaboration with other professionals and implement treatment strategies based on scientific nursing theory that promotes physical, psychosocial, emotional, and spiritual well‑being. Familiar with the concepts and needs of patients/families who are facing death and dying.
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $75,620 to $113,430 annually based on full‑time employment. We comply with all minimum wage laws as applicable.
