Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) for Hospice in the North Kansas City area, Missouri. The role is full-time, 40 hours per week, with 8-hour shifts and a Staff employment type. A $10,000 first-year success bonus is offered. Travel to patient homes within the Kansas City region is required.

The RN will provide end-of-life care, collaborate with an interdisciplinary team, and document care in the EMR. Active Missouri RN license and 2+ years’ experience preferred.

Qualifications

• Active RN license in the state you work
• Two years of RN experience; hospice experience preferred
• Driver’s license and ability to travel ~20% of the workday

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes
• Perform comprehensive patient assessments and end-of-life care planning
• Educate and support patients and families through the end-of-life journey
• Maintain thorough, compliant EMR documentation
• Work independently while collaborating with an interdisciplinary team
• Travel to patient homes within the assigned territory
