Job summary

Brighton Hospice is seeking a dedicated Registered Nurse (RN) to provide compassionate, patient-centered hospice care in patients' homes, SNFs, and assisted living facilities. You will collaborate with an interdisciplinary team to build individualized care plans while maintaining manageable caseloads and strong patient relationships.

Qualifications

• Current Registered Nurse (RN) license in good standing in the state of practice.
• A passion for providing exceptional, patient-centered hospice care.
• Strong collaboration and communication skills with interdisciplinary care teams.
• Ability to work independently while effectively managing your daily schedule.
• Excellent time management and organizational skills to balance patient visits, documentation, meetings, and urgent needs.
• Comfort learning and utilizing electronic medical record (EMR) systems.
• Reliable transportation, a valid driver's license, and current auto insurance to travel throughout the service area.

Responsibilities

• Provide compassionate, individualized hospice care wherever patients call home—including private residences, skilled nursing facilities, and assisted living communities.
• Collaborate with an interdisciplinary team of nurses, Social Workers, Chaplains, CNAs, and volunteers to develop care plans.
• Maintain lower clinician caseloads and higher staff-to-patient ratios to allow meaningful time with patients and families.
