Job summary

Traditional Home Health Care in Lackawanna County, PA seeks an experienced Registered Nurse to support hospice admissions and community liaison responsibilities. The role involves start-of-care visits, care planning, and coordination with families, physicians, and nursing supervisors.

Ideal candidates will have PA RN licensure, strong assessment and documentation skills, and the ability to travel to case assignments. Weekly pay and comprehensive benefits are offered.

Qualifications

  • Current license as a Registered Nurse in Pennsylvania.
  • Excellent assessment skills, documentation skills and ability to meet deadlines.
  • Must have a current driver's license.
  • Ability to travel to case assignments.
  • Strong interpersonal skills and clear verbal/written communication.
  • Acute care and/or home health experience is preferred.

Responsibilities

  • Perform start of care visits on hospice patients and admit patients, completing clinical documentation as required.
  • Initial evaluations for hospice appropriateness.
  • Obtain consents during initial hospice discussions with patients and families.
  • Assess patient status and develop a care plan with nursing supervision, physician, patient and family.
  • Coordinate hospice referrals including obtaining records/orders/diagnosis.
  • Maintain electronic hospice contracts.
  • Assist with transition of care between palliative and hospice programs.
  • Provide hospice philosophy teaching.
  • Coordinate care audits with SNFs.

Job responsibilities include, but not limited to the following:


  • Perform start of care visits on hospice patients and admit patients to Traditional Home Health Care, completing clinical documentation as required

  • Initial evaluations for hospice appropriateness
