Job summary

Summit Home Care & Hospice seeks a registered nurse to provide home health and clinical nursing care in Columbus, OH, and surrounding areas. The role blends field visits with occasional office training, requiring patient assessments, care planning, and medication administration.

You will regularly re-evaluate patient needs, collaborate with physicians and families, and participate in on-call rotations. Valid RN license, CPR, and a driver’s license are essential.

Qualifications

Responsibilities

• Perform initial patient and family assessment to determine home care needs
• Conduct complete physical assessments and histories of illnesses
• Regularly re-evaluate nursing needs and adjust care plans
• Develop and revise care plans with patient/family involvement
• Provide medications and treatments as prescribed by physician
• Coordinate with clinic manager and coordinate patient care
• Prepare clinical notes and update physician at required intervals

• Job Location: Columbus, OH
• Salary Range: $75,000.00 – $85,000.00 per year
• Sign‑On Bonus: $5,000 – $10,000 based on previous hospice experience
• Hours: Monday – Friday; Standard Business Hours & occasional on‑call rotation
• Location/Territory: Columbus, OH & Franklin County
• Work Setting: On the road for patient visits & training at the local office or remotely

Responsibilities

Patient Care

• Completes an initial assessment of patient and family to determine home care needs
• Provides a complete physical assessment and history of current and previous illnesses
• Regularly re‑evaluates patient nursing needs
• Initiates the plan of care and makes necessary revisions as patient status and needs change
• Uses health assessment data to determine nursing diagnosis
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions, including the patient and the family in the planning process
• Initiates appropriate preventive and rehabilitative nursing procedures
• Administers medications and treatments as prescribed by the physician
• Provides direct patient care as defined in the State Nurse Practice Act
• Counsel the patient and family in meeting nursing and related needs
• Provides health care instructions to the patient as appropriate per the assessment and plan of care
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient
• Acts as case manager when assigned by the clinic manager and assumes responsibility to coordinate patient care for assigned caseload

Communication

• Prepares clinical notes and updates the primary physician when necessary and at least every 60 days
• Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required
• Communicates with community health related people to coordinate the care plan

Additional Duties

• Participates in on‑call duties as defined by the on‑call policy
• Ensures that arrangements for equipment and other necessary items and services are available
• Instructs, supervises, and evaluates home health aide care provided every two weeks

Qualifications

• Graduate from an accredited school of nursing, associate or bachelor’s degree
• Valid/active nursing license
• One year of previous experience as an RN required
• Must be a licensed driver with an automobile in good working order and insured according to the organization's requirements
• Valid CPR certification required
• Recent TB test required
• Prior home health care or acute care experience preferred
• Management experience preferred but not required
• Excellent observation, verbal and written communication skills, problem‑solving skills, basic math skills; nursing skills per competency checklist
• Requires prolonged or considerable walking or standing and considerable reaching, stooping, bending, kneeling or crouching
• Must be able to lift, position or transfer patients and lift supplies and equipment
• Must have visual acuity and hearing to perform required nursing skills
