Role Overview

This position involves providing specialized nursing care to hospice patients and their families, focusing on pain and symptom management, emotional, spiritual, and cultural support. The nurse assesses patient needs, develops and implements individualized care plans, collaborates with a multidisciplinary team, and evaluates patient progress to ensure quality, evidence-based care. When serving as a case manager, the nurse coordinates all aspects of patient care, advocating for patient-centered, culturally sensitive support.
