Job summary

Agape Hospice in Tucson, AZ seeks an experienced Registered Nurse to plan, organize and direct home-based hospice care. The RN must hold an active Arizona license and a valid driver’s license to travel within the community.

You will assess patients, develop care plans, administer medications, educate families, and coordinate services with the clinical team. This role emphasizes compassionate, patient-centered care in home settings, with strong communication, autonomy and teamwork.

Qualifications

• RN with current licensure to practice professional nursing in the state.
• Arizona Driver’s License or ability to obtain one.
• Auto-insurance valid and in good standing.

Responsibilities

• Completes initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Administers medications and treatments as prescribed by the physician in the plan of care.
• Develops a care plan based on nursing diagnosis and initiates appropriate nursing procedures.
• Coordinates patient care and acts as Case Manager when assigned by Clinical Supervisor.
• Counsels patient and family in meeting nursing and related needs, with health education as appropriate.

JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Patient Care

• Completes an initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Provides a complete physical assessment and history of current and previous illness(es).
• Provides professional nursing care by utilizing all elements of nursing process.
• Assesses and evaluates patient’s status by: Writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, participating in revising the plan of care as necessary.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
• Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan.
• Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
• Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

Position Qualifications

• Valid Arizona Driver’s License.
• Automobile in good working order.
• Proof of current automobile insurance.
• RN with current licensure to practice professional nursing in the state.
• Able to read, write and speak English fluently.
• Thorough working knowledge of core nursing skills and is able to apply such knowledge of practices, techniques, procedures, and principles to ensure the highest quality of nursing care possible is provided to patients.
• Ability to assess patient’s needs and direct care to meet those needs Ability to deal tactfully with customers and the community.
• Demonstrates good communications, negotiation, and public relations skills.
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
