Job summary

TEEMA is seeking a compassionate Registered Nurse (RN) to provide high-quality home health and hospice care in Ames, IA. The role emphasizes independent work, patient-family relationships, and coordination within an interdisciplinary team.

You will perform Medicare home health visits, conduct assessments, and develop plans of care, with an emphasis on education, symptom management, and thorough documentation. Homecare Homebase (HCHB) experience is preferred.

Qualifications

  • Active and unrestricted RN license in the state of employment.
  • Minimum 1 year of RN experience in acute care, oncology, long-term care, hospice, or home health.
  • Current American Heart Association BLS.
  • Valid driver's license and reliable transportation; willing to travel within a 60-mile radius.

Responsibilities

  • Complete approximately 6–8 patient visits per day.
  • Perform initial and ongoing nursing assessments.
  • Provide skilled nursing care in patients' homes.
  • Develop and implement individualized Plans of Care.
  • Monitor patients for changes in condition and communicate concerns to physicians.
  • Provide patient and caregiver education, including medication management, dressing changes, oxygen use, and symptom management.
  • Coordinate care with physicians, LPN/LVNs, CNAs, and other interdisciplinary team members.
  • Provide support and education to patients and their families.
  • Maintain timely and accurate clinical documentation.
  • Follow Medicare, Medicaid, infection-control, and regulatory guidelines.
  • Participate in staff meetings and quality improvement activities.
  • Participate in the agency's on-call rotation when required.

Registered Nurse (RN) – Home Health / Hospice
Position Overview

We are seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality care to patients in their homes. This position is ideal for an RN who enjoys working independently, building relationships with patients and families, and coordinating care as part of an interdisciplinary healthcare team.

The RN will complete Medicare Home Health visits, perform patient assessments, coordinate plans of care, provide education and symptom management, and maintain accurate clinical documentation.

Homecare Homebase (HCHB) experience is preferred.

What You'll Do
