Tufts Medicine Care at Home is seeking an experienced Hospice RN Case Manager to serve as the case manager for the interdisciplinary team, delivering quality hospice nursing care per patient plans in Malden, MA and surrounding areas.

The role requires an active RN license, CPR (or BLS within 30 days depending on hire date), and 1 year of hospice/home health experience, with opportunities for professional growth and ongoing education.

Qualifications

Responsibilities

• Provide hospice nursing care using current principles of palliative care and symptom management.

Job Profile SummaryThis role focuses on providing professional and nonprofessional nursing care services in accordance with physician orders. In addition, this role focuses on performing the following Home Care Nurse duties: Assesses, plans, implements, and evaluates the care needs of a designated group of patients or provides nursing care and identified clinical support tasks within the scope of practice for achievement of the patient’s plan of care as directed. Provides professional nursing care to patients. Requires a nursing license. A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs. Candidates must hold an active RN license in the state where services are provided and may not work in states where they are not licensed. If additional state licensure is required, it must be obtained within sixty (60) days2. Current CPR Certification.3. One (1) year of hospice and home health experience.4. If hired prior to 9/28/2026, CPR Certification is required upon hire. If hired after 9/28/2026, BLS Certification is required within thirty (30) days of hire.Preferred Qualifications:1. Provides hospice nursing care to patients and families utilizing current principles of palliative care and symptom management to produce hospice outcomes.2. Remains competent in technical nursing skills (i.e. IVs, Gtubes, etc.)6. Provides effective patient and family teaching.7. Communicates with physician and Tufts Medicine Care at Home staff regarding changes in patient’s condition.8. Understands family dynamics and works effectively within various types of family systems.9. Provides peers with support and mentoring if appropriate.11. Demonstrates the ability to act as a patient and family advocate. Provides information to families about all available resources.12. Accurately addresses need for additional services.13. Performs adequate number of nursing visits and on call/triage necessary to provide superior hospice care under Tufts Medicine Care at Home standards.15. Communicates patient reports and status changes concisely and effectively.17. Updates Care Plan, Medication Profile and HHA Treatment Plan routinely and as patient condition changes.18. Completes all nursing documentation per procedures and nursing standard of practice including assessments, interventions, responses to interventions, communications, verbal orders, etc.20. Manages caseload efficiently and effectively.21. Involves Clinical Manager and Medical Director in situations appropriately.22. Follows guidelines for weekend and evening coverage.26. Pursues standards of excellence in performance as a hospice case manager.27. Works with Clinical Manager to identify opportunities for improvement in performance and works to address them.28. Proactive in increasing knowledge of hospice care and standards of practice.29. Assists in hospice education in community.Physical Requirements:1.
