Job summary

Agape Hospice Care in Woodstock, GA is seeking a registered nurse to administer skilled nursing care to patients in their homes. You will coordinate with the interdisciplinary team, educate families, and ensure high-quality end-of-life care.

The role requires an RN license, CPR, and at least 2 years of clinical RN experience, preferably in hospice or home health. A strong patient/family focus and proficiency with computers are essential.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred
• Current CPR certification
• Proficient skill using a computer; Microsoft Office experience preferred
• Strong interpersonal and communication skills; ability to support patients and families
• Demonstrate ethical and compliant nursing practice

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs and ADLs.
• Implement and revise the plan of care (POC) with the IDT as needed.
• Educate patients and families on disease process, self-care, end-of-life care; train staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare progress notes showing progress toward goals.
• Coordinate patient/family services with the IDT and prioritize needs.
• Inform physicians of changes in patient needs and outcomes of care.
• Determine scope and frequency of services based on acuity and needs.
• Assess caregiver ability and provide support as needed.
• Communicate information using current processes and technology.
• Aid admission and re-certification criteria for hospice care.
• Perform on-call responsibilities and provide on-call service.
• Other duties as assigned.

Position Summary: Primary functions are to administer skilled nursing care for clients of all ages in their
place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency;
and assumes the responsibility for coordination of care.
Essential Duties:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, functional, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural/spiritual, verbal /non-verbal.
• Implement the individualized POC and recommend revisions to the plan as necessary.
• Consulting with and educating the patient/family regarding disease process, self-care techniques, end-of-life care, nutrition and dietary needs. Providing training to other staff as needed.
• Initiating appropriate preventive and rehabilitative nursing procedures.
• Preparing clinical and progress notes that demonstrate progress toward established goals.
• Coordinating all patient/family services and prioritization of needs with the members of the IDT.
• Use of case management approach and referring to other services as needed.
• Informing physician and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care.
• Determining scope and frequency of services needed based on acuity and patient/family needs.
• Assessing ability of the caregiver to meet the patient’s immediate needs upon admission and throughout care.
• Evaluating own needs for support and using identified systems to meet the need
• Communicating information using current process and technology available to the organization.
• Applying specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient’s eligibility.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Other duties as assigned.
