Three Oaks Hospice, Inc. is seeking a compassionate Registered Nurse to administer skilled nursing care for clients in their homes and coordinate care with the interdisciplinary team.

This role involves evaluating patient needs, developing and revising the care plan, and ensuring high-quality end-of-life care for patients and families. Qualifications

• Graduate with an accredited Associate or Bachelor's degree in Nursing.
• Current RN license and valid state driver's license.
• Two years of RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills.
• Ability to support patients and families and cope with emotional stress.

Responsabilités

• Initial and ongoing comprehensive assessments of patients in their home environment to determine care needs.
• Coordinate care with the interdisciplinary team and coordinating services with patient/family and referring agencies.
• Develop and implement the plan of care and revise as needed.
• Educate patients and families about disease processes, self-care, end-of-life care, nutrition and diet.
• Provide training to other staff as needed.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes showing progress toward goals.
• Coordinate patient/family services and prioritize needs with the IDT.
• Refer to other services as needed and inform physician of changes in patient needs.
• Determine scope and frequency of services based on acuity and needs.
• Assess caregiver ability to meet patient needs and provide support resources.

Position Summary

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence coordinate care with the interdisciplinary team patientfamily and referring agency; and assumes the responsibility for coordination of care

Essential Duties

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical functional psychosocial and environmental needs and ADLs ie risk for grief culturalspiritual verbal non verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Consulting with and educating the patientfamily regarding disease process self care techniques end of life care nutrition and dietary needs
• Providing training to other staff as needed
• Initiating appropriate preventive and rehabilitative nursing procedures
• Preparing clinical and progress notes that demonstrate progress toward established goals
• Coordinating all patientfamily services and prioritization of needs with the members of the IDT
• Use of case management approach and referring to other services as needed
• Informing physician and other personnel of changes in the patients needs and outcomes of intervention while evaluating patientfamily response to care
• Determining scope and frequency of services needed based on acuity and patientfamily needs
• Assessing ability of the caregiver to meet the patients immediate needs upon admission and throughout care
• Evaluating own needs for support and using identified systems to meet the need
• Communicating information using current process and technology available to the organization
• Applying specific criteria for admission and re certification to hospice care to establish appropriate levels of care and the patients eligibility
• Perform on call responsibilities and provide on call service to patientsfamilies as assigned
• Other duties as assigned

Qualifications

• Graduate of an accredited Associate or bachelors degree in Nursing
• Current state license as an RN and current state drivers license
• Two years experience as an RN in a clinical care setting; previous hospice or home health experience preferred
• Current CPR certification
• Proficient skill using a computer is required; Microsoft office suite experience is preferred
• Strong interpersonal skills and ability to provide effective support to the patient and family
• Demonstrate skill in nursing practice ability to cope with family emotional stress and tolerance of individual lifestyles
• Strong communication and organizational skills and high standard of ethics and compliance
