Job summary

UnitedHealth Group in Cleveland, OH is seeking a Per Diem Registered Nurse Case Manager to deliver the hospice Plan of Care and ensure quality patient care. This role requires providing compassionate, collaborative support to patients and families in a hospice setting.

You will perform initial evaluations within 48 hours, reassess at least every 15 days, document progress, and coordinate the plan of care with the IDG to meet patient needs.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• MS: substitution of 3 years LPN for RN experience.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Make the initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Identify physical, psychosocial, and environmental needs; reassess at least every 15 days.
• Initiate and coordinate the plan of care with accurate clinical notes about the patient's condition.
• Document problems, goals, interventions, and patient/family response to hospice care.

As the Per Diem Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluate personal and professional performance and makes necessary changes to increase productivity and quality of care delivered. Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care.

Primary Responsibilities:

• Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral
• Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days
• Initiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition
• Documents problems, appropriate goals, interventions, and patient/family response to hospice care
• Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily

State Specific Requirements:

* MS: At least one-year full-time experience as an RN. However, three (3) years full-time clinical experience in a healthcare setting as an LPN may be substituted for the one-year full time experience as an RN

Preferred Qualification:

• Current CPR Certification or ability to complete within 90 days of hire
