Amedisys, part of the Optum family, is seeking a Per diem Hospice Registered Nurse to provide compassionate, patient-centered home health care in Blackstone, VA. You will assess needs, coordinate individualized care plans, manage symptoms, educate families, and collaborate with the clinical team.

Required qualifications include a current RN license for the state, CPR certification, and a valid driver's license with reliable transportation. Preferred: 1+ year of nursing experience.

As a Per diem Hospice Registered Nurse, you will provide compassionate, patient-centered care by assessing needs, coordinating individualized care plans, managing symptoms, educating families, and collaborating with the clinical team.

Primary Responsibilities:

• Performs initial nursing assessment on new admissions
• Completes, coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff
• Regularly communicates patient progress to the clinical manager, physician, and care team
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies
• Records pain/symptom management changes/outcomes as appropriate
• Educates families on patient condition, needs and care
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so
• Performs on-call responsibilities and on-call services to patients/families as assigned
• Performs other duties as assigned

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $71,640 to $107,460 annually based on full-time employment. We comply with all minimum wage laws as applicable.
