Job summary

Ascend Hospice in Overland Park, KS seeks an RN on-call (weeknights) to assess and document patient needs, coordinate with the Hospice Nurse, and deliver care per physician orders. You will support family communications, participate in Interdisciplinary Team meetings, and ensure supplies and documentation are maintained.

Requirements include an active RN license in KS, 1 year of clinical experience, and hospice or LTC exposure preferred.

Qualifications

• Hold a Valid and current RN license within the state of operations.
• 1 year of clinical experience.
• Long Term Care/Home Care/Hospice experience preferred.
• Strong verbal and written communication skills.
• Able to work as part of an inter-disciplinary team.

Responsibilities

• Observes and documents the physical, mental and emotional needs of the patient and family on an ongoing basis.
• Consults with the Hospice Nurse regarding the patient's status and symptom management.
• Delivers physical and psychological care as prescribed by the attending physician and supervised by hospice staff.
• Monitors and documents patient/family input and response to care.
• Maintains continuity of patient care through ongoing communication with families and other health care professionals.
• Participates in Interdisciplinary Team Meetings and other assigned meetings.
• Maintains home care supplies for patient care management.
• Collects documentation and paperwork as requested by the Hospice Nurse.
• Assists with clinical billing as deemed necessary.
• Participates in orientation of new hospice aides.

Hours: Monday–Thursday, 4:00 PM – 2:30 AM

TheHospice Registered Nurse On Call (RN Weeknights) will be responsible for, but are not limited to:

• Observes and documents the physical, mental and emotional needs of the patient and family on an ongoing basis

• Consults with the Hospice Nurse regarding the patient's status and symptom management at appropriate levels

• Delivers physical and psychological care as prescribed by the attending physician, as supervised by the Hospice Nurse and as approved by the IDC team

• Monitors and documents patient/family input and response to care

• Maintains continuity of patient care through ongoing communication with families and other health care professionals

• Participates in Interdisciplinary Team Meetings and all other meetings as assigned

• Maintains sufficient and appropriate home care supplies for adequate patient care management

• Collection of documentation and paperwork as requested by the Hospice Nurse.

• Assistance with clinical billing as deemed necessary

• Participates in orientation of new hospice aides as assigned

Education:

Associate's Degree in a related field

Professional Experience:

• Hold a Valid and current RN license within the state of operations
