Job summary

Buckeye Hospice and Palliative Care Services in Freemont, Ohio, is seeking a full-time registered nurse to perform initial and comprehensive nursing assessments and to implement and adjust the plan of care for hospice patients in home settings and facilities.

The role requires an active RN license, a valid Ohio drivers license, and 14 years of hospice or home health experience. On-call rotation and a benefits package are included, with a schedule that supports holidays and weekends.

Qualifications

• RN License is required.
• Valid driver2s license and reliable transportation.
• Minimum of 1 to 4 years as an RN working with hospice services or home health.

Responsibilities

• Provides the initial assessment and comprehensive nursing assessment of patient needs.
• Implements the plan of care and reevaluates and revises the plan of care relative to the patient's nursing needs to provide appropriate, comprehensive and responsive goals and interventions.
• Communicates the patients progress with the patient/caregiver and other disciplines as well as directs, supervises and instructs direct care staff members and nonprofessional hospice aide staff to deliver quality care and coordinates delivery of care services.
• Assessment and care of Hospice patients in nursing facilities and home settings.
