Job summary

The role requires an active RN license and 1–2 years in hospice or palliative care, with strong clinical skills and excellent communication.

Qualifications

• Valid RN license in the practicing state.
• 1–2 years of hospice, palliative care, or related nursing experience.
• Strong clinical nursing skills and knowledge.
• Excellent communication with residents, families, and staff.
• Ability to prioritize tasks in a fast-paced setting.
• Understanding of hospice regulatory requirements.
• Compassionate, rapport-building bedside manner.

Responsibilities

• Conduct thorough patient assessments covering physical, emotional, and spiritual needs.
• Develop and implement individualized care plans with patients and families.
• Administer medications, manage pain and symptoms, perform procedures as needed.
• Monitor progress, document changes, and respond to treatment effects.
• Educate patients and families on disease progression and care options.
• Collaborate with physicians, therapists, and other staff to coordinate care.
• Provide emotional support and facilitate end-of-life discussions.
• Coordinate with hospice team to ensure continuous care.
• Advocate for patient preferences and values.
• Maintain accurate records per regulatory standards.
• participate in on-call rotations as required.
• Perform other duties as assigned.
