Autumn Hospice is hiring an RN Case Manager in Massachusetts. The role involves conducting initial assessments, coordinating care plans, and working with an interdisciplinary team to provide quality end-of-life care.

Qualifications

• Current State RN license.
• Minimum of two years of professional nursing experience; hospice experience preferred.
• Valid driver's license and reliable transportation.
• Willingness to travel.
• Excellent verbal and written communication skills.

Responsibilities
