Job summary

Compassus is seeking a Hospice Registered Nurse / RN Case Manager in Illinois. You will enable patients to spend quality time with their loved ones and coordinate compassionate care through assessments, planning, and ongoing support.

You will build trusting relationships with patients and families, educate caregivers on at-home care, and collaborate with the care team to deliver comfort, peace, and dignity—ensuring compassionate, person-centered service.

Qualifications

• Active RN license in the state you work.
• Two years of registered nurse experience, hospice experience preferred.
• Valid driver’s license and current CPR/BCLS certification.

Responsibilities

• Enable patients to spend quality time with loved ones and activities they enjoy.
• Build trusting, compassionate relationships with patients and families.
• Educate caregivers on home-based care and patient needs.
• Collaborate with the care team to provide comfort, peace, and dignity.
