We’re looking for a skilled and empathetic Registered Nurse with a passion for providing compassionate care at the end of life. The role is a Hospice Registered Nurse Case Manager to join our team and provide patients and families with the support and care they need during life’s most challenging moments.

Key Responsibilities
  • Provide skilled nursing care to hospice patients, focusing on pain management, symptom relief, and emotional support.
  • Manage a caseload of patients, develop personalized care plans, and coordinate care with the hospice team.
  • Educate patients and families about the hospice process, end‑of‑life care, and coping strategies.
  • Assess patient needs regularly, report changes in condition, and adjust care plans accordingly.
  • Offer emotional support to families and provide guidance throughout the hospice journey.

Detailed Responsibilities
  1. Complete the initial, comprehensive, and ongoing assessment of patients and families and provide direct or supervised nursing services based on a developed plan of care.
  2. Develop an individualized plan of care in collaboration with the interdisciplinary team, patient, and family based on assessment, needs, and goals.
  3. Communicate with the physician regarding patient’s needs, response to treatment, and changes; obtain/receive physician orders as required.
  4. Work closely with IDG members and family/caregiver to provide holistic physical, psychosocial, emotional, and spiritual services, including bereavement support.
  5. Demonstrate positive interpersonal relations with all agency members.
  6. Effectively demonstrate the mission, vision, and values of the agency on a daily basis.
Job Specifications
  • Education: Graduate of an NLN accredited school of nursing and current license to practice as a Registered Nurse in the state; Bachelor’s degree preferred.
  • Licensure / Certifications: Current RN license in NC, NC driver’s license with insured vehicle, and current American Heart Association CPR certification.
  • Experience: Minimum one year’s clinical experience; hospice experience preferred.
  • Technical/Motor Skills: Working knowledge of Word, E‑mail, typing 30 wpm, basic knowledge of home‑health billing.
  • Interpersonal Skills: Ability to work independently and as part of an interdisciplinary team.
  • Physical Requirements: Intermittent walking, standing, sitting, lifting, and supporting patients.
  • Mental Abilities: Ability to concentrate with large volumes of paperwork, manage deadlines, exercise good judgment, and prioritize workload independently.
  • Essential Sensory Requirements: Keen observation skills.
  • Exposure to Hazards: May be exposed to virus, disease, and infection from patients and specimens.
  • Hours of Work: Monday – Friday, with on‑call responsibilities and holidays as needed.
